Accurate, complete information helps us respond quickly. All information is held in confidence and used solely to assess this tenancy and the creditworthiness of the applicant and any guarantors.

One signed PFS for each principal and each prospective guarantor (pages 6–8).

For corporations/LLCs/partnerships: most recent YTD Balance Sheet and Profit & Loss Statement, plus the two most recent business tax returns.

Signed authorization page (final page) for each principal/guarantor. Sensitive ID information is collected only on that page.

Articles of incorporation/organization and authorization naming the officer(s) authorized to sign a lease.

AUTHORIZATION

Disclosure & Authorization. In connection with my application to lease commercial space, I authorize Realty 1 Partners and its agents, successors, and assigns ("Landlord") to obtain a consumer report and/or investigative consumer report about me from a consumer reporting agency, and to obtain and verify information including credit history, criminal and eviction (unlawful detainer) records, banking and trade references, and rental and employment history, for the purpose of evaluating this tenancy and any personal guaranty. I understand a report may be obtained before, and at any time during, the tenancy. I authorize banks, creditors, landlords, and references to release relevant information to Landlord. A photocopy, fax, or electronic copy of this authorization is as valid as the original. I certify that the information in this application is true and complete to the best of my knowledge, and I acknowledge that any false statement may result in denial of this application or termination of any resulting lease. This authorization is for tenant-screening and creditworthiness purposes only and creates no obligation on Landlord to extend credit or lease space.

Privacy & data protection (Texas). Realty 1 Partners maintains reasonable safeguards to protect sensitive personal information (including Social Security and driver's license numbers) from unauthorized use or disclosure and, when no longer needed, destroys such records by shredding, erasing, or otherwise making them unreadable, consistent with the Texas Identity Theft Enforcement and Protection Act (Tex. Bus. & Com. Code ch. 521). Information is used solely to evaluate this tenancy and is not sold. This form is provided for general business use and is not legal advice; consult counsel before use.
